01
Vaccination History
Please gather any available vaccination records from childhood through adulthood, including
international records.
If records are unavailable, no action is required—our office provides all
necessary vaccinations on-site in accordance with USCIS requirements.
02
Vaccine Requirement
You need only one dose of each missing vaccine to complete your I-693. We
will advise you on finishing any series afterward. Children need all vaccines appropriate
for their age.
MMR (measles, mumps, rubella)
Two documented doses, or a blood test showing immunity to all three, or one dose at
the exam.
Varicella (chickenpox)
Two documented doses, a reliable history of chickenpox, a blood test showing
immunity, or one dose at the exam.
Hepatitis B (through age 59)
A documented complete series, a blood test showing immunity, or one dose at the exam.
Tdap (tetanus, diphtheria, pertussis)
A documented primary series with a dose in the last 10 years, or one dose at the
exam.
Polio
Written records of a complete childhood series (3 doses). If records are not
available, one dose of polio vaccine (IPV) is given at the exam. Blood testing is
not a practical option, as commercial labs do not test for all three poliovirus
types required by USCIS.
Flu
Required during flu season when the vaccine is available.
COVID-19
No longer required.
